Don’t miss out on care support in Ludlow

TIME is running out for people in Ludlow and south Shropshire to register their ongoing need for special support.

Shropshire County Primary Care Trust (PCT) is encouraging anyone who thinks they may have been eligible for NHS Continuing Healthcare to come forward.

NHS Continuing Healthcare is the package of support funded by the NHS, to help care for a patient’s physical or mental health needs that have arisen as a result of disability, accident or illness.

Individuals assessed as having certain level of care needs may receive NHS Continuing Healthcare.

It is not dependent on a particular disease, diagnosis or condition, or on who provides the care or where that care is provided.

If an individual’s overall care needs show that the primary need is a health need, the person should be eligible for NHS Continuing Healthcare.

The Primary Care Trusts is currently reviewing applications for previously unassessed periods of care, which require assessment for NHS Continuing Healthcare.

The PCT is encouraging people who think they may have been eligible for NHS Continuing Healthcare, between April 1 2011 – March 31 2012, to contact the appropriate team before the deadline of 31 March 2013.

Individuals should contact their local Primary Care Trust for further information on how to notify the PCT of a request for assessment of eligibility.

It is important people come forward as soon as they can, as applications received after the deadlines may only be considered if there are exceptional circumstances for doing so.
